Your first appointment at Village Pointe Oral Surgery will include a consultation explaining your diagnosis and treatment options. We encourage you to contact our office today for more information and to schedule an appointment with our oral surgeon in Omaha, Nebraska.
Please provide the following information at the time of your consultation:
- Your surgical referral slip and any X-rays, if applicable
- A list of current medications
- Bring the necessary completed forms for your medical or dental insurance, if applicable

X-Rays
You may request that your dentist or physician forward any X-rays to our office. If there is not enough time, please pick them up and bring them to our office. If additional X-rays are needed, they can be taken at our office.
IMPORTANT
: All patients under 19 years of age must be accompanied by a parent or guardian at the consultation visit with Dr. Michael Shnayder.
A preparatory consultation and physical examination is mandatory for patients who will have IV anesthesia administered for surgery. Do not eat or drink for eight hours prior to your surgery. You will need to have someone to drive you home.
Please let us know if you have a medical condition that may be a concern for surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
